When I graduated dental school at OHSU in 1992 and started my practice, little did I know that seventeen years later I would also be a naturopathic doctor. I became a naturopathic physician in order to be able to help more people with their health issues. For you, the patient, our clinic offers a place of caring and professionalism for the whole person, in a small office setting.

We are realists. There are costs and insurances and everyone has a different financial scenario and stress level. We try to work with you so you can get the care you need with a plan and in a fashion that best meets your own unique situation.

Our Team

Dr. Azizi
Dr. Azizi
Dr. Azizi has been in the health care field for 25 years. She graduated from OHSU dental school in 1992 and from the National college of Natural Medicine in 2009 with academic honors and honors in research. She also holds a certificate in nutrition and one in functional gastroenterology from Dr Steven Sandberg-Lewis and has taken over 300 post-graduate hours in functional neurology from the Carrick Institute for Graduate Studies.

Shelly Tam
Shelly (Cielito) Tam has been a great dental assistant for 20 years. She and Dr Azizi started the practice together in 1997. Shelly is well versed in all aspects of dentistry as she is also a dentist, graduated from Centro Escolar University School of Dentistry in the Philippines. Besides the back office (dental assisting), she is also well familiar with the front office work.
